INSTRUCTIONS: TEAM ENTRY & FREE AGENT PARTICIPATION

Three Options for obtaining a team roster or free agent form:

1. Download the roster or form via a word file intramural team roster or free agent form, save it & email the roster as an attachment to the Coordinator of Intramurals @ nharris@millersville.edu
2. Select the PDF file below & print team roster or free agent form:
   1. Bring the completed roster or form to a Student Memorial Center (SMC) Sign-Up Day (check intramural calendar) or bring to the Campus Recreation Office in Pucillo Gym, Room 210, upstairs along the track.
3. Receive a team roster or free agent form @ the SMC Sign-Up Days in the SMC banking lobby OR the Campus Recreation Department in Pucillo Gym, Room 210, upstairs along the track (Check intramural calendar)

ADDITIONAL INSTRUCTIONS

• ATTEND MANDATORY Captain's Meeting on Wednesdays (Unless Otherwise Noted) @ 9:00 p.m. @ Pucillo Gym, Room 209, upstairs along the track
• $10 Forfeit fee payments CANNOT be accepted @ captain's meetings!!!
• Forgot your roster, get one @ the MANDATORY captain's meeting
• $10 forfeit fee MUST BE PAID @ the SMC bank by 4:30 p.m.;
• NO FORFEIT FEE RECEIPT, NO PLAY!!!
• BRING the forfeit fee receipt and roster to the captain’s meeting
